Firefighter Physicals

A Healthcare Providers Guide to Firefighter Physicals is available from the Pennsylvania State Fire Commissioner's website. It is recommended that you take this printout to your doctor for your annual physical. The printout can be found here. Annual physicals are covered by the Relief Association.

Baldwin Borough Firefighter Relief Association has also made arrangement with St. Clair Occupational Medicine Center to provide physical exams to the Baldwin Borough Firefighters. Please call St. Clair Occupational Medicine Center, 412-942-7115 to schedule your appointment. When scheduling your appointment please inform the staff if the physical is “Routine” or “DOT”.  It is recommended that all drivers get the "DOT" exam.

If your medical history includes diabetes, sleep apnea, cardiac or mental health issues please inform the staff as there will be additional information needed for the doctor to sign off on your physical. Please complete the form below and bring them, along with you photo ID, to your appointment. If you are having a "DOT" physical please wear or bring a pair of shorts.

There is no lab work involved so there is no need to fast. You can download a form to fill out ahead of time here.

St. Clair Occupational Medicine Center is located in Bethel Park across for the St. Thomas More Catholic Church in the Hospital’s Outpatient Center. The office is in Suite 100, first office on the rights where we share space with the St. Clair Hospital Urgent Care Center.
